Norman Rehabilitation & Medical Center offers therapy sessions of 28
days that include 6 days of therapy and 1 day off per week.

COST INFORMATION:

The applicable cost for the therapy session is as follows:
$6,650.00 US Dollars for non EU citizens.

If you wish to bring another person, there will be an additionaI charge of
$37.00 USD for adult, and $34.00 USD per child per day.

4,850.00 (Euro) for EU citizens.
For EU citizens, the extra charge per person will be:
23.00 Euro for adult, and 21.00 Euro per child per day.
The cost includes the accommodations and three daily meals for the
patient and the adult coming with him or her.

There is a $1000.00 USD or 1000.00 Euro deposit required to reserve a spot per therapy
session according to the patient's applicable region. In order to ensure availability, please request a
reservation at least three months prior to the date you want to start Spide therapy. The balance is due
and payable one week before starting the session if it is paid by wire transfer or on the first day of
therapy if it is paid in cash. We accept the payment either in US dollars or Euros.

We strongly suggest that you make the final payment in CASH due to the fact that the banks and
money exchange agencies have a transaction fee to exchange any document such as travelers checks.
In this way you can save money. Firma Norman can not accept cashier's checks or money orders.

CANCELLATION POLICY:

***If you need to cancel or reschedule your session(s) before 90 days of the starting date of your
therapy there will be no deduction from your deposit.

***If you need to cancel or reschedule your session(s) 90-60 days before the starting date of your
therapy there will be a deduction of $250.00

***If you need to cancel or reschedule your session(s) 60-30 days before the starting date of your
therapy there will be a deduction of $500.00

***If you need to cancel or reschedule your session(s) less than 30 days of the starting date of your
therapy there will be a deduction of $1000.00

More Information

**It is important for our Orthopedic doctor and therapists that you give us all the necessary
information regarding the condition of the patient in order to be able to give you a complete
evaluation. If your child has had X-Rays, tomography, EMG (Electromiogram), Gait test or MRI
(Magnetic Resonance Imaging) bring them with you.

**Remember to have ALL the medications taken by your child and yourself to avoid any interruption
in your medical treatment.

**We need to know if the patient, the parent or the care giver that will be with the patient during the
therapy session have any allergies.

**Feel free to let us know if there is any allergies to foods. We will do our best to prepare the meals
upon your needs.

**Our therapy center can accomodate our patients according to their particular needs on our rooms
with one, two or three beds.

**Every session begins on a Sunday and ends on a Sunday (28 day session) and each patient's therapy
treatment will end on a Saturday. Thus, if possible, you will depart on Sunday morning. Your arrival
and departure schedule is up to you, and of course we will be available to answer questions or give
suggestions if you need them. We understand the uncertainties that are sometimes associated with
international traveling. We are flexible and will accommodate your needs to the best of our abilities.
